Dr.Himanshu Sheth shows the uses and steps in the creation of a Lab Produced Milled Bar Prosthesis on which a variety of partical denture designs can be fabricated.

Lab Produced Milled Bar Prostheses

Bar prostheses are indicated for restoration of largely edentulous cases where a selected few sound abutments are available. These abutments are root-treated and reduced to about 2-3 mm above gingival level and prepared to recieve a radicular post and coping. A bar is then fabricated by use of a milling unit with an angulation parallel to the soft tissue undercuts, and milled with an alignment groove. A custom-cast female component is then prepared. The prostheses is luted into place and then a pickup impression is taken to fix the position of the female component in relation to the male. Pour the impression with die-material (Type IV) without applying any separating material inside the female attachment. The female would hereby be embedded firmly on the model.

A sressbreaker effect can be incorporated by applying a few coats of spacer (nailvarnish) to the top surface of the male prior to the pickup impression and removed therafter.

The denture is then processed as usual and when devested from the model shows the female component correctly embedded in the denture. The denture is now ready for delivery.
